Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: (a) Francisco's Framing, Inc. at Lot 19 19th Avenue West, Williston, ND 58801: On or about October 29, 2011, the employer did not ensure that employees were protected from falling when framing a single family home in the following instances: 1. An employee was walking on the exterior top plate, with approximately a 12' fall hazard; 2. The stairway hole to the basement was not covered or guarded, measuring 42" by about 6', with approximately a 9' fall hazard; and 3. The two basement window wells were not covered, each measuring 38" x 41" with approximately a 5' fall hazard. Conditions exposed employees to fall hazards of up to approximately 12'.

Condition exposed employees to electrical shock hazards.
