At the worksite located on Lot Number: 5103-1, high hazard forklift operations were being performed without the use of seat belts. Seat belts in the forklift truck are a component part of an operator restraint system that is designed to reduce the incidence and severity of injuries to the operator in the event of a tipover accident, a hazard to which forklift trucks are particularly susceptible, and that the failure to wear the seat belt that is provided in the forklift truck increases the risk of injury to the operator in the event of such an accident.      Page 4 of the Sky Trak Model 9038 Owners/Operators Manual state, "Always adjust the seat and fasten the seat belt securely before you start the engine."
