29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: (a) The hazard communication training provided by the employer did not include specific information on the substances in their work area generated from the welding operations, in that the employees in the welding area did not receive training on the hazards associated with iron oxide, manganese, and zinc oxide.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year: (a) The OSHA 300 log for 2009 was not accurate and complete, in that a checkmark was not placed in Column J, i.e. Other recordable cases, for the entries in Rows 26 through 33, which resulted in the incorrect total for Column J.
