Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) On or about April 28, 2022, the employer did not ensure that the Commercial Electric relocatable power strip used to power the overhead light and fan at the workstation was used in accordance with the instruction included in the listing or labeling in that, the relocatable power strip was being used a permeant extension.

The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a) On or about April 28, 2022, the employer did not ensure a path to ground was maintained for employees utilizing a Commercial Electric 4ft LED Linkable Shoplight (120 Volts, 60Hz) in that the lights power cord was missing the grounding pin.
