29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) On or about November 22, 2011, the employer did not ensure that employees were protected from falls in excess of 10 feet in the central area of the roof (central air vents), and falls in excess of 20 feet on the east and west ends of the roof (cooling towers area) while working or walking near the openings without fall protection.

29 CFR 1910.66(f)(5)(ii)(J): Acess to and egress from a working platform, except for those that land directly on a safe surface, was not provided by stairs, ladders, platforms, and runways conforming to the provisions of Subpart D of this part. a) On or about November 22, 2011, the employer did not ensure that employees wer protected from falls while accessing the window washing platform (basket). The employees would access the platform by stepping on the floor of the roof onto rail (20 inches high) from the rail onto carriage of the window washer (17 inches high) from the carriage onto the ledge of the building (parapet 36-40 inches high) and from the parapet into the basket (36 inches wide).
