29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not have each affected employee use the types of personal protective equipment that would protect the affected employee from the hazard(s) identified:  Bob Popp Building Services Inc, at 600 17th Street, Denver, CO: On 4/9/12, when Bob Popp Building Services Inc had assessed the workplace hazard and determined that a hazard was present, the employer did not have each affected employee use the type of personal protective equipment that would protect the affected employees from the hazard identified.  Employees set up and moved a Spider Modular Platform System suspended scaffold during window washing duties on the approximate thirty story tall South Tower of the Dominion Plaza office building.  The employer had identified the fall hazard.  The employer did not ensure that employees utilized fall protection while setting up or moving the suspended scaffold from the roof of the thirty story building.  This condition exposed employees to an approximate thirty story fall hazard.
