On or about November 29, 2011, at the above addressed jobsite, employees were exposed to the hazard of contact with asbestos containing material (ACM) while conducting roofing tear off operations. The roofing material contained 6% chrysotile asbestos in the black (tar) mastic. The employer did not institute a training program for employees who performed Class II asbestos operations.
