(b) In the Eastside Health & Rehabilitation exposure control plan there was no information that described how post-exposure follow-up would be accomplished. (c) In the Eastside Health & Rehabilitation exposure control plan there was no information that described how employee training would be accomplished. 29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. (a) On or about December 1, 2011, the employer did not record correctly the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2011. 1. On or about November 1, 2011, an employee received a work restriction due to a back injury when pulled by a resident. 2. None of the cases listed on the 2011 OSHA 300 form have an entry in column G, H, I, or J as required. 3. The description of injury was not complete for all cases numbered 1 through 20. For example case number 1 has a description of injury of 'hand injury' but does not describe which hand nor the object or substance that directly injured the person, nor what the injury was.

(a) On or about December 1, 2011, the employer did not record correctly the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2010. 1. Entries for cases numbered 1, 2, 3, and 4 are marked as being cases with job transfer or restriction and there was no entry in column L to indicate the number of restricted work days. 2. The description of injury was not complete for cases numbered 1, 2, 3, 4, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, 19, 20, 21, 22, and 23. For example case number 1 has a description of injury of 'hip strain' but does not describe the object or substance that directly injured the person. 3. There was no number of days of job transfer or restriction entered in column L for cases 1, 2, 3, or 4, which were marked as cases of injury with restrictions.
