On or about December 7, 2013, an employee received a back injury with 2 days of lost time and there was no entry in column K of the number of days lost.        3.     On or about December 20, 2013, an employee received a back injury which resulted in lost time and the description of injury was incomplete.        4.     On or about December 20, 2013, an employee received a shoulder and hip injury which resulted in 4 days of restricted work and there was no entry in column L of the number of days of restricted work.
