29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: (a) On or about December 23, 2013, the employer did not record correctly the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012. 1. On or about November 24, 2013, an employee received a sharps puncture injury and the case was not recorded as a privacy case and column J was not marked. 2. On or about December 7, 2013, an employee received a back injury with 2 days of lost time and there was no entry in column K of the number of days lost. 3. On or about December 20, 2013, an employee received a back injury which resulted in lost time and the description of injury was incomplete. 4. On or about December 20, 2013, an employee received a shoulder and hip injury which resulted in 4 days of restricted work and there was no entry in column L of the number of days of restricted work.
