29 CFR 1910.157(e)(2):     Portable fire extinguishers were not visually inspected at least monthly:    a) Bldg. 94, mezzanine, column F35 - On or about 4/10/12, a fire extinguisher missed its monthly visual inspection in January and March 2012.    b) Bldg. 94, mezzanine, adjacent to electrical panel 7-14-LP-2-14 - On or about 4/10/12, a fire extinguisher missed its monthly visual inspection in January and March 2012.    c) Bldg. 94, first floor, in hallway outside of supervisor's office - On or about 4/10/12, a fire extinguisher missed its monthly visual inspection in March 2012.    d) Bldg. 94, mezzanine, in hallway outside of FISC office - On or about 4/10/12, a fire extinguisher missed its monthly visual inspection in January and March 2012.

29 CFR 1910.303(b)(1):     Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):    a) At Bldg. 466, lunchroom, below bulletin boards - On or about 12/6/11, an electrical outlet servicing a snack dispensing machine was not covered to prevent inadvertent contact with exposed 120VAC electrical service wiring.

29 CFR 1910.334(a)(1):     Flexible electric cords were fastened with staples or otherwise hung in such a fashion as could damage the outer jacket or insulation:    (a) At Bldg. 94, sheet metal work area adjacent to South men's restroom - On or about 2/16/12, a pendant electrical outlet's cord was tied in a knot so that the insulation was damaged.

29 CFR 1910.303(g)(1):     Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    a) At Bldg. 94, adjacent to stairs - On or about 12/6/11, a pallet jack was stored in the area in front of 115V switch boxes and blocked access to switch boxes L4.1, L4.2, L4.3, L4.4, L4.12 and L4.13.    b) At Bldg. 94, adjacent to stairs - On or about 12/6/11, the area in front of a 115V electrical panel labeled 7-6-LP-1-6(NW) was used to store an electric fan.    c) At Bldg. 94, Quickie shop, adjacent to south Lunchroom - On or about 12/7/11, the area in front of a 115V electrical panel was used to store an electric fan.    d) At Bldg. 94, Electrical panels 440 MA3-13 and MA3-14 - On or about 12/7/11, the area in front of four electrical panels were used to store a rolling cart with equipment.    e) At Bldg. 94, first floor, in hallway outside of supervisor's office - On or about 4/10/12, access to a switch box was blocked by an electric cart.

1910.303(g)(2)(i)(B):    Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or by suitable permanent, substantial partitions or screens so arranged that only qualified persons will have access to the space within reach of the live parts:     a) Bldg. 466, lunch room, NMCI electrical panel between refrigerator and server - On or about 12/6/11, an electrical panel did not a have panel cover so that the panel could not be locked and was accessible to non-qualified personnel.    b) Bldg. 94, Electrical panels 440 MA3-13, MA3-14, MA3-17, MA3-18, MA3-19, MA3-20 - On or about 12/7/11, the 440 V electrical panel covers had latches but could not be locked. In addition, panel MA3-17 latch cover was missing. Live parts were accessible to non-qualified personnel.
