29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: At Talo Verde, Lot #8, 1st and 2nd floors, an employee working near loose 8" masonry blocks was not provided with safety foot wear. Employee was exposed to the hazards of loose 8" masonry blocks crushing the feet.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At Talo Verde, Lot #8, 2nd floor, an employee was operating a Hilti TE 706-AVR demolition hammer without the use of eye protection. Employee was exposed to flying concrete chips contacting the eye.
