29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap: a) On or about October 31, 2011, and at times prior, employees were exposed to injury in that, the flexible cords between the transformer and the mud tanks had been spliced and covered with electrical tape.
