29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that were not part of the permanent wiring of the building or structure and that were in use by personnel did not have ground-fault circuit-interrupter protection for personnel: BEHIND MOBILE HOMES: On or about July 30, 2014 the employer did not ensure that extension cords were connected to an operational ground-fault circuit interrupter, exposing employees to an electrical shock hazard.

The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: BEHIND MOBILE HOMES: On or about July 30, 2014, an extension cord powering water and sump pumps did not have a ground pin. The path to ground was not continuous and effective, exposing employees to electrical hazards.
