29 CFR 1910.36(b) (1): At least two exit routes were not available in the workplace to permit prompt evacuation of employees and other building occupants during an emergency: a) On or about December 8, 2011, at the above located workplace employees were required to work in the kitchen. The emergency exit doors on the west and south side of the building were locked. Only one evacuation route was available to employees. The employees did not have at least two exit routes available and thereby were exposed to the hazards associated with fires and smoke inhalation.

29 CFR 1910.36(c) (1): Each exit discharge did not lead directly outside or to a street, walkway, refuge area or open space with access to the outside: a) On or about December 8, 2011, at the above located workplace employees were required to work in the kitchen. The emergency exit door on the south side of the building lead to a locked gated area measuring approximately 30 feet by 15 feet. The employees were thereby exposed to the hazards associated with fires and smoke inhalation.

The kitchen's west and south emergency exit doors were locked. The employees did not have the keys, tools or special knowledge to open the exit doors and were exposed to the hazards associated with fires and smoke inhalation.
