29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system.    a)  Rick Hammond Construction, LLC on or about December 12, 2011, at the address of 501 Park Ave. Elyria, OH: The employer failed to ensure employees were protected from a fall while working from a ladder jack scaffold.   Employees were exposed to a near 18 fall to the ground below during roofing operations of a residential structure. No form of fall protection was in place.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a) Rick Hammond Construction, LLC on or about December 12, 2011, at the address of 501 Park Ave. Elyria, OH: The employer did not ensure employees were protected from falls while removing old roofing material from a residential type structure.  Employees were exposed to a 20ft fall to the ground below.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     a) Rick Hammond Construction, LLC on or about December 12, 2011, at the address of 501 Park Ave. Elyria, OH: The employer did not ensure the side rails of the 20 foot fiberglass ladder were extended at least three feet above the landing. Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
