29 CFR 1910.305(e)(1):  Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures:    (a)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure each energized conveyor outlet boxes, 110V, located on the pipe conveyors inside the shop were weather rated for damp or wet conditions. Boxes had water dripping onto them from the conveyor assembly.  This condition exposed the employees to electrical shock hazards.    (b)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure each energized conveyor outlet boxes, 110V, located on the pipe conveyors outside the shop were weather rated for damp or wet conditions. Boxes were exposed to outdoor weather conditions.  This condition exposed the employees to electrical shock hazards.

One drive shaft end rotating at high rpm was exposed at the west rear end of the blast cleaning machine. This condition exposed employees to laceration and amputation hazards.    (b)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11,  guarding was not provided to protect employees from hazards created by the rotating chain on the side of the Wheelabrator (blast cleaning machine) A138363. The rotating chain is exposed through a square opening of approximately 6 X 12 on the west side of the blast cleaning machine. This condition exposed employees to laceration and amputation hazards.    (c)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, guarding was not provided to protect employees from hazards created by a extruding rotating horizontal shaft on the Wheelabrator (blast cleaning machine) A138363. The shaft is located towards the front west end of the blast cleaning machine. This condition exposed employees to laceration and amputation hazards.

(b)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not provide a standard railing and toe board on the main floor at the end of pipe inspection, by the where the floor drops down by several inches. The walking-working surface is not even. This condition exposed employees to fall hazards.

(b)	 Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure that employees using a power cord connected to the Blast Cleaning Machine were protected from electrical hazards in that, the insulated metal conduit protecting employees from coming into contact with the inner conductors was damaged.   The  conduit was open approximately 1/2 inch exposed the inner power cord. This condition exposed employees to burns, shock, and electrocution hazards.

(b)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure that the flexible cord being used to power the boom box radio near the end of the pipe inspection was from an approved receptacle outlet. A flexible cord was traced from a junction box on the conveyor outlet to the radio resting on a portable computer cabinet. This condition exposed the employees to electrical shock and electrocution hazards.    (c)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure that the flexible cord being used to power the computer near the end of the pipe inspection was from an approved receptacle outlet. A flexible cord was traced from a junction box on the conveyor outlet to the computer resting on a portable computer cabinet. This condition exposed the employees to electrical shock and electrocution hazards.    (d)	Commercial Resins Company, 8100 E. 96th Ave, Henderson, CO 80640: On and before 12/14/11, the employer did not ensure that the flexible cord being used to power the computer speakers near the end of the pipe inspection was from an approved receptacle outlet. A flexible cord was traced from a junction box on the conveyor outlet to the HP computer speakers resting on a portable computer cabinet. This condition exposed the employees to electrical shock and electrocution hazards.
