29 CFR 1904.29(b)(6): The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent:      (a)  On or about November 14, 2011, an employee received a puncture injury from a needle and the case was recorded using the employee clock number in Column B.      (b)  On or about February 20, 2011, an employee received a puncture injury from a needle and the case was recorded with no information in Column B.      (c)  On or about November 9, 2008, an employee received a puncture injury from a needle and the case was recorded using the employee name in Column B.      (d)  On or about April 9, 2008, an employee received a puncture injury from a needle and the case was recorded using the employee name in Column B.
