29 CFR 1910.147(c)(4)(ii)(B):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rule, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section:    (a)  Warehouse:  On or about December 8, 2011, and at time prior thereto, the established energy control procedures did not address how to effectively de-energize equipment with multiple sources of energy such as the Cram-A-Lot TVB Baler.    Notes:      (1) To abate this violation, the employer must develop specific written procedures to effectively shut down, isolate, block and secure each machine or piece of equipment with multiple sources of energy.     (2) Pursuant to 29 CFR 1903.19, the employer must submit copies of the written energy isolation procedures.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct an annual or more frequent inspection of the energy control produce to ensure that the procedure and requirements of the standard were followed:    (a)  Warehouse:  On or about December 8, 2011, and at times prior thereto, the employer did not implement procedures to periodically inspect the energy control procedures to ensure that the procedures and/or requirements of this standard were followed.    Notes:      (1) To abate this violation, the employer must conduct reviews of each energy control procedure to ensure the procedure will effectively isolate each energy source on the particular piece of equipment being reviewed.    (2) Pursuant to 29 CFR 1903.19, the employer must submit a written certification that the energy control procedures have been reviewed.

29 CFR 1910.147(c)(7)(i): The employers training program did not include all the training elements listed under Items A-C of this section:    (a)  Warehouse:  On or about December 8, 2011, and at times prior thereto, the employer's training program did not ensure employees were equipped with the knowledge to recognize the type and magnitude of the energy, the hazards of the energy to be controlled, and the method to control the energy.    Notes:    (1) To abate this violation, the employer must ensure the training contains the following elements: Each authorized employee shall received training in the recognition of applicable hazardous sources, the type and magnitude of the energy available in the work place, and the methods and means necesssary for energy isolation and control; Each affected employee shall be instructed in the purpose and use of the energy control procedures; andall other employees who work operations are or may be in the area where energy control procedures may be utilized, shall be instructed about the procedures, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.    (2) Pursuant to 29 CFR 1903.19, the employer must submited the names of each employee trained and the date(s) of the training.

29 CFR 1910.147(c)(5): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware was not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources.
