29 CFR 1926.451(e)(1): Scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were used. Crossbraces were used as a means of access. a) On the north side: On 12/6/2011, and at times prior thereto, sub contractors employees were not provided with a safe means of accessing the second and third level decks of the tubular welded frame scaffold. Note: Abatement certification and documentation ARE required. The employer, Ormond Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) which was contained in OSHA inspection Number 311582761 Citation number 1, item number 2 and was affirmed as a final order on 06/26/2009, with respect to a workplace located at 900 Environmental Way, Idaho Falls, ID 83401.

29 CFR 1926.451(g)(1): Employee's on a scaffold platform more than 10 feet (3.1 m) above a lower level were not protected from falling to a lower level. Fall protection for scaffolding must meet requirements specified in 29 CFR 1926.451(g)(1)(i) through (vii): a) On the north side scaffold: On or about 12/6/2011, and at times prior thereto, sub contractor's employees working on the tubular welded frame scaffolding at heights of approximately 11.5 feet were not protected by guard rails and end rails. Note: Abatement certification and documentation are required. The employer, Ormond Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1) which was contained in OSHA inspection Number 311582761 Citation number 1, item number 1d and was affirmed as a final order on 06/26/2009, with respect to a workplace located at 900 Environmental Way, Idaho Falls, ID 83401.

29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was possible, precautions against such damage were not taken: a) ) On the southeast corner: on or about 12/6/2011, and at times prior thereto, the 500 gallon LP Gas tank on the concrete slab was not protected from damage by the GEHL powered industrial truck when loading materials on the scaffold.
