29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) Tocha Bridge, Talfofo- On or about December 05, 2011 and at times prior thereto, the Caterpillar Rough Terrain Forklift Truck TH62, Equipment number 479, used two rebar steel pins to hold the forks in place. The forklift was used to move materials in the area.
