29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. On or about September 16th, 2011, an employee fell waist deep into the electro-clean dip tank while attempting to find a loose part in the tank. No temporary covers or equivalent methods were used to protect employee(s) while working above the dip tanks. The employee was hopitalized with third degree burns as a result of the accident. No Abatement Certification or Abatement Documentation Required.

The lanyard selected for employee(s) to use while working above a dip tank was too long to ensure that employees would not have any part of their body fall into the tank(s) containing hazardous substances.
