Woodbridge Rd., Monclova, Ohio, the employer did not ensure that employees were protected from falls while installing frame ridges on a low sloped surface located on a single family residential structure. Employees were exposed to falls of approximately 19.2 feet to the ground below.

Woodbridge Rd., Monclova, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection systems while employees were installing a ridge pole on a residential structure. Employees were exposed to falls of approximately 19.2 feet to the ground below, which could result in permanent disability or death.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) Dave Stoll Construction, Monclova, Ohio: On or about January 28, 2011, at address Lot 46, N. Woodbridge Rd., Monclova, Ohio, the employer failed to ensure that a 10 ft. A-framed stepladder was opened during use. The ladder was closed and leaning against the structure while employee used it to access the upper portion the roof. Employees were exposed to fall hazards due to the improper placement and use of this ladder.

29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step: a) Dave Stoll Construction, Monclova, Ohio: On or about January 28, 2011, at address Lot 46, N. Woodbridge Rd., Monclova, Ohio, the employer failed to ensure that the top of a 6 ft. A-framed stepladder was not used as a step by employees. The employees were using the top of the ladder to access the roof of a residential structure. The employees utilizing the ladder in this condition were exposed to fall hazards of approximately 10 feet.
