At the facility - On or about December 25, 2011, A US Postal Service employee suffered a heart attack at work on December 25, 2011 at approximately 10:30 PM, the employer was notified of the employee's death December 26, 2011 at 12:15 am, and did not report the fatality to the Depart of Labor - OSHA until the following day, December 27, 2011.
