29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about May 10, 2016, and at times there prior to, on the main floor, the employer did not ensure that only stable or safely arranged loads were handled. 1) A pit operator on the main floor, was towing equipment that was not safely arranged, exposing employees to struck by hazard. 2) Employees were operating tow equipment that was missing the connector attachment. Operators exposed other employees to struck by hazards due to unstable loads and from loads not safely arranged. 3) Operators routinely move rolling equipment with pallet trucks, exposing employees to struck by hazards from unstable loads.

29 CFR 1910.178(p)(1): The employer did not ensure that when powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe were taken out of service until restored to safe operating condition. On or about May 10, 2016 the employer did not ensure that a damaged powered industrial truck was taken out of service after it was identified that the connection point to tow unit MO5 was damaged. Employees were exposed to struck by hazards from the inability to keep loads secured during the towing process.
