Listed or labeled equipment was not used in accordance with instructions included in the listing or labeling:  Dock 3: On December 28, 2011 and at times prior thereto, an Intermat relocatable power tap was used as a substitute for permanent wiring to energize a dock lamp.  The Army & Air Force Exchange Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(b)(2) which was contained in OSHA Inspection Number 315607580, Citation Number 01, Item Number 004, issued on 09/07/2011, with respect to a workplace located at 240 West Fitzgerald Bldg 6001 at Edwards Air Force Base, CA 93524 which became final order on 09/30/2011.

When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electrical equipment safe had been made:  Dock 3: On December 28, 2011 and at times prior thereto, the Intermat relocatable power tap insulation at the male end of the power cord had been pulled from it's fitting and the internal conductors were exposed.  The Army & Air Force Exchange Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.334(a)(2)(ii) which was contained in OSHA Inspection Number 315607580, Citation Number 02, Item Number 001, issued on 09/07/2011, with respect to a workplace located at 240 West Fitzgerald Bldg 6001 at Edwards Air Force Base, CA 93524 which became final order on 09/30/2011.

The Log of Work-Related Injury and Illness and the Summary of Work-Related Injuries and Illnesses (OSHA 300 and OSHA Form 300A or their equivalent) was not provided within four business hours to an authorized government official upon request:  On December 29, 2011, this Compliance Safety and Health Officer requested to OSHA 300 Logs and the OSHA 300A Summary Forms for calendar years 2008, 2009, 2010, and 2011. These documents were not provided until January 18, 2012.
