29 CFR 1910.303(g)(1)(i)(B): The width of working space in front of the electric equipment was not the width of the equipment or 762 mm (30 in.), whichever was greater:  The front of the back trailer: The width of the working space in front of the electrical panel was not at least 30 inches as there was a display table located next to the wall, under the panel. Employees were exposed to the hazards of electricity.

29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm.  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being locked into walk-in freezers and temperatures of about 32 degrees Fahrenheit.  The storage area on the north side of the store: A padlock was installed on the exterior of the door to a Bally Engineering Structures, Inc. walk-in freezer to prevent the theft of cups, napkins and boxes stored inside. The freezer was not in operation, but access was through another freezer located next to it. The adjacent freezer was in operation and reduced the temperature to near freezing. The freezer door cannot be opened from the inside when it was padlocked shut. The freezer was about 8 feet long by 8 feet wide by 7 feet high. The door mechanism that allowed the door to be opened from the inside was removed and defeated by the padlock.  Among other methods, one feasible and acceptable method to correct this hazard is to:

29 CFR 1910.36(g)(2): An exit access was not at least 28 inches (71.1 cm) wide at all points:  (a) The back trailer area: The aisle widths between the clothes racks and the display shelves were not at least 28 inches wide as they were about 24 inches wide, exposing employees to the hazards of fire.  (b) The front trailer area: The aisle widths between the ice cream freezer and the boxes of bottled water stored on the floor were not at least 28 inches wide as they were about 19 inches wide, exposing employees to the hazards of fire.  THE ARMY & AIR FORCE EXCHANGE SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.36(G)(2) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 190312, CITATION NUMBER 1, ITEM NUMBER 3, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT BULDING 940, LANGFORD LAKE ROAD, FORT IRWIN, CALIFORNIA.
