29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On 1/6/2012, the following machines were not properly safeguarded.  a) The two Amada and one Clausing horizontal bandsaws had used portions of the saw blades that were not guarded.  b) The Goodway lathe did not not have a chuck guard.  c) The Birmingham milling machine did not have a chuck and tooling guard.  d) The 10 Warner and Swasey automatic multi spindle machines did not have interlocks at the operator splash/barrier guard. Additionally, there were openings as larger as 13" by 18" with 25" to the point of operation. Also the rotating bar stock spindle casings were open at the ends including no guarding for the rotation of the spindles.  e) The magnetic interlocks for the doors of the Kia L028, L029, and L031 CNC lathes were by passed.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      On 1/5/2012, at the Hitec Turn 235II, Hitachi Seiki L001 and the Hitec Turn 255 there were unsecured electrical receptacle junction boxes with knockouts attached to flexible cords.     NOTE: Knockout boxes were designed to be secured to a fixed/permanent structure. When knockout boxes are used on the ends of flexible cords, employees gripping the box to attach or disconnect a plug may dislodge the circular scored metal portion in the box wall causing it to come in contact with the hot terminal. The result may lead to an electric shock.
