29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. Materials or equipment were placed, either permanently or temporarily, within the exit route. The exit access went through a room that could be locked, to reach an exit or exit discharge.     On or about April17, 2017, the employer did not ensure that the emergency exits located at each end of the main facility by the employees front entrance points, were free of obstructions that could impede the safe and efficient evacuation of employees in the event of an emergency.  Access to the emergency exit doors were obstructed by black metal gate doors that are closed and are not a push through type of door.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    On or about April 17, 2017, the employer failed to establish and implement a written program for the control of hazardous energy.  Employees who are not trained as authorized are exposed to accidental re-start of machinery while performing maintenance work on various CNC machines and blade change outs on horizontal band saws with energy sources to include but not limited to electrical and hydraulic.
