Live parts to which an employee may be exposed was not deenergized before the employee works on or near them, and the employer did not demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations.      a.) Chiller Building, Coleman National Fish Hatchery - On or about November 04, 2012 and at times prior thereto, the 480 volt electrical panel was not deenergized prior to an employee working on the equipment.

The employer did not ensure the employee used safe work related practices such as lock out/tag out procedures.

Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145.    a.) Coleman National Fish Hatchery, Chiller Building - On or about November 04, 2011 and at times prior thereto, the 480 volt electrical panel did not have a sign or symbol warning employees regarding arc flash.
