29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery,equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    a) PM Bay #6 - On or about January 11, 2012, employee(s) were required to use PM Bay #6 on a regular basis to perform their assigned duties. The concrete ground floor in PM Bay #6 contained a damaged floor drain cover that created a floor opening approximately 11 1/2 inches in length and 6 inches in width. Employee(s) were thereby exposed to a potential trip hazard.    b) Outside wash bay overhead door #10 south side - On or about January 11, 2012, employee(s) use the grounds outside wash bay overhead door #10 (south side) on a regular basis. The grounds outside wash bay overhead door #10 (south side) had a missing inspection cover that created a hole approximately 11 1/2 inches in width. Employee were thereby exposed to a potential trip hazard.    c) Bay #3 - On or about January 11, 2012, employee(s) were required to use Bay #3 on a regular basis to perform their assigned duties. The concrete ground floor in Bay #3 contained a damaged floor drain cover that created a floor opening  approximately 13 inches in length and 1 1/2 inches in width.  Employees were thereby exposed to a potential trip hazard.
