29 CFR 1910.119(l)(1):     The employer shall establish and implement written procedures to manage changes (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process.      On or about December 5, 2011, the 25%-35% Caustic Line from from PU-1219 to DR-1157 leaked at a flange. It was identified that an envelope gasket had been installed. Original design specfications for this type of service requires spiral wound gaskets to be used. An MOC was not conducted for the change in the gasket type, style, torque values, and metallurgy.
