29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections shall be performed to assure that equipment is installed properly and consistent with design specifications and the manufacturer's instructions. On or about December 5th, 2011, the employer did not conduct appropriate checks and inspections to assure equipment was installed properly and consistent with design specifications and manufacturers instructions. When new gaskets were installed on the 25%-35% Caustic Line, proper torqueing values, methods, and/or procedures were not followed.

29 CFR 1910.119(l)(1): The employer shall establish and implement written procedures to manage changes (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. On or about December 5, 2011, the 25%-35% Caustic Line from from PU-1219 to DR-1157 leaked at a flange. It was identified that an envelope gasket had been installed. Original design specfications for this type of service requires spiral wound gaskets to be used. An MOC was not conducted for the change in the gasket type, style, torque values, and metallurgy.
