Production Area - On or about January 18, 2012, the employer's written energy control program did not include the specific procedural steps for shutting down, isolating, blocking and securing machines for the employees required to perform maintenance and/or services on equipment such as, but not limited to the press brake (AMADA F03015), press brake (AMADA RG50), laser cutter (Amada FO-3015), and the Robotic Welding Machine (GENESYS Serial # S0989). Employees were thereby exposed to the hazards of moving and rotating parts.

Entire Facility - On or about January 18, 2012, employees were required to perform maintenance and repairs for production equipment such as, but not limited to press brake (AMADA FO3015), press brake (AMADA RG50), laser cutter (AMADA FO3015), and the robotic welding machine (Genesys Serial # S0989). The employer had conducted periodic inspections of energy control procedures at least for year 2011.
