29 CFR 1910.179(j)(2)(iii): Hooks with deformation or cracks. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�° twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section. a. Crane Periodic Inspections/Jib Cranes - On or about May 24, 2016, employees were required to move loads using hoisting equipment and daily visual inspections were not conducted. Employees were thereby exposed to struck by hazards. 1. Building # 1/the CM - chain hoist's hook was stretched out and missing its safety latch.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding. a. Welding Building # 4 - On or about May 24, 2016, employees were required to conduct welding operations in areas where welders were not entirely screened on all sides. Employees (welders and non welders) were thereby exposed to the hazards of welding arcs.
