OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a fall hazard due to the self-retracting lifeline having a knot tied into the synthetic webbed strap.     a.)  At ODW Logistics, there was an employee who was operating a high lift order picker truck with a self-retracting lifeline as a fall arrest system to protect against a fall hazard.  The employee's self-retracting lifeline had a knot tied into the synthetic webbing which significantly reduced the load capacity if a fall were to occur.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a)  During the inspection, it was observed at the battery charging station that an electrical plug did not have strain relief for its flexible cord, which supplied 480 volts to a fork lift battery charger.  A flexible cord in this condition exposes employees to a shock and/or fire hazard.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    Located at ODW Logistics in Columbus OH:    a)  On the 2009 OSHA 300 Log, the employer did not include all information that was required in Column F.           1.  On Case #1, there was no indication of the object/substance that directly fractured the left ankle.          2.  On Case #4, there was no description of the injury that was received to the left heel.
