29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative:    a) Facility: On January 3, 2012 and times prior thereto the employer had not developed and implemented an energy control program for the employees who perform activities which include, but are not limited to, scheduled and unscheduled maintenance on machines and equipment that include, but are not limited to the MAN ROLAND print press, the HEIDELBERG Windmill and Die Cutter, and WOLTENBURG Cutter, nor had they provided awareness training to those employees who may be affected by these activities in their work areas. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Hand Bindery room:  On January 3, 2012 and times prior thereto a guard was not in place at the rear area of the shrink wrap machine where a pulley assembly was located and the point of operation was exposed.     b) Bindery area: On January 3, 2012 and times prior thereto the vintage HEIDELBERG Windmill and Diecutter machines both had exposed flywheels, low to the floor, located on the side of each machine, with storage shelves or a walkway close by.
