Sunset St., Longmont, CO 80501: On and before December 13, 2011, the employer did not ensure all forklift operators were trained in operation of powered industrial trucks in that the operators training consist of a combination of formal instruction, practical training, and evaluation of the operators performance in the workplace.  Operator training consisted of on-the-job practical training only, operators have not been certified, and operators have not received re-evaluations every three years.  This condition exposed employees to the hazard of unsafe fork-lift operations.
