OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     On or about January 10, 2012 and times prior, an employee was picking up production water from a wellhead. When the employee exited the well site traversing a roadway the brakes of the Mac truck identified as truck #58 did not function as designed, resulting in the truck losing control and rolling over.    Abatement Verification:  Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").
