29 CFR 1910.179(b)(3):     Crane(s) were modified and such modifications and the supporting structure were not checked thoroughly for the new rated load by a qualified engineer or the equipment manufacturer:    (a) Chips Spinning Area: Abell-Howe Wall//Jib Crane (Crane #2) was not checked and rerated for lifting capacity after holes were drilled into the top of the jib crane beam and a steel plate bolted to the top of the beam. The crane hoist was then attached to the eye bolt. The modification exposed crane operators and other employees to a crushing hazard.
