29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. A. Buckeye Excavating & Construction, Inc. worksite located near 2503 Strubb Rd., Sandusky, Ohio: On or about January 24, 2012, the employer did not ensure employees wore eye protection while operating a rotary saw to cut precast concrete pipe. No eye protection was being worn exposing the employees to eye injuries from flying debris.

Buckeye Excavating & Construction, Inc. worksite located near 2503 Strubb Rd., Sandusky, Ohio: On or about January 24, 2012, the employer did not ensure the trench box was less than two feet from the bottom of the trench. The trench box was approximately 3.40 feet above the base of the trench exposing the employee to crushing injuries resulting from the trench box shifting vertically.

29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. A. Buckeye Excavating & Construction, Inc. worksite located near 2503 Strubb Rd., Sandusky, Ohio: On or about January 24, 2012, the employer did not ensure the the swivel hook attached to the excavator had an operable safety latch while hoisting precast concrete structures and pipe into a trench. The throat of the hook was open exposing employees to the hazard of falling loads. Buckeye Excavating & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.251, which was contained in OSHA inspection number 311607964, citation number 1, item number 1 and was affirmed as a final order on March 30, 2009, with respect to a workplace located at Bowling Green, OH.

Buckeye Excavating & Construction, Inc. worksite located near 2503 Strubb Rd., Sandusky, Ohio: On or about January 24, 2012, the employer did not ensure employees were protected from cave-in/collapse hazards while preparing to install precast concrete pipe and related structures in a trench approximately 11.40 feet deep. The east end of the trench was near vertical exposing the employees. No end plate was being used. Buckeye Excavating & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652, which was contained in OSHA inspection number 311608962, citation number 1, item number 1 and was affirmed as a final order on June 9, 2009, with respect to a workplace located at Bowling Green, OH.
