29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    On or about March 14, 2018, at the work site located near the address of 727 Lime City Rd. Rossford, OH: The employer failed to ensure material was placed at least two feet from the edge of the excavation. Employees operating a hydraulic boring machine from in the excavation were exposed to sliding/falling spoil located on the south edge of the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).     On or about March 14, 2018, at the site located near the address of 727 Lime City Rd. Rossford, OH: The employer failed to ensure employees were adequately protected from cave-ins while employees were performing work tasks (boring) from in a 6 ft. 3 in. deep unprotected trench.
