On or about March 14, 2018, at the work site located near the address of 727 Lime City Rd. Rossford, OH: The employer failed to ensure material was placed at least two feet from the edge of the excavation. Employees operating a hydraulic boring machine from in the excavation were exposed to sliding/falling spoil located on the south edge of the excavation.
