29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer's energy control program was inadequate in that it allowed for multiple lockout devices (locks) to be keyed the same.  Thus not affording complete employee protection from potential energy start up.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Maintenance employees perform work on machinery in the facility. Employees utilize lock out tag out but did not have documented procedures to follow.  The employees are exposed to an unexpected startup of a machine if all sources of energy were not locked out.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.253(b)(4)(iii):     Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour.     Oxygen and Acetylene cylinders stored in the maintenance area are not stored at least 20 feet apart or with a fire rated wall that would separate the cylinders. Employees are exposed to a fire hazard while working in the facility.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:    Employees who perform live voltage testing up to 480V during preventative maintenance on machinery did not have the proper gloves, head protection, and appropriate face shields to prevent against electrical shock and burns.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
