29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) Quick's Hoe & Landscape Service, Inc., @ 6784 Oneida St., Commerce City, CO 80022: On 1/26/12, the employer did not ensure that employees were protected from cave-ins. Employees conducted fire hydrant extension and replacement for three hours in an excavation which measured approximately 6 feet deep without the aid of a cave-in protection system. This condition exposed employees to struck-by hazards.

This condition exposed employees to struck-by hazards.

Employees conducted fire hydrant extension and replacement for three hours in an excavation which measured approximately 6 feet deep without the aid of a cave-in protection system. This condition exposed employees to struck-by hazards.
