29 CFR 1926.602(d) NOTE: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.178(l) of this chapter.      29 CFR 1910.178(l)(1)(ii):     The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.      On January 31, 2012 at a worksite located at 5223 US Hwy 251, Peru Illinois, an employee was operating a Genie model GTH-844 rough terrain forklift and the employer did not train and evaluated the operator.        No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.    On January 31, 2012 at a worksite located at 5223 US Hwy 251, Peru Ilinois, an employee was unloading roofing materials on a low slope commercial roof without fall protection and was exposed to a fall hazard of approximately 9 feet.    Brigadier Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 31076953, citation number 01, item number 01, and was affirmed as a final order on August 6, 2007, with respect to a workplace located at 413 Saulk Trail Road, Richton Park, Illinois.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
