29 CFR 1926.150(c)(1)(vi):  A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:      a)  A fire extinguisher was not provided where employees were using a 50 pound propane tank to fuel a roofing torch.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      a)  Employees engaged in roofing activities were exposed to falls of approximately 18 feet.    Brigadier Roofing Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 31314037490, citation 1, item 3, and was affirmed final order on September 1, 2010, with respect to a workplace located at 4435 Fox Valley Center Drive, Aurora, IL 60504, and OSHA inspection number 190441, citation 2, item 1, and was affirmed final order on April 10, 2012, with respect to a workplace located at 5223 US Hwy 251, Peru, IL 61354.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
