29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million: On or about January 30, 2012, workers at the Millard Refrigerated Services, Inc., facility located in Geneva, Illinois, were exposed to carbon monoxide at the following levels: a. General Laborer, was exposed to an 8-hour time-weighted average of 127.9 ppm, based upon percent carboxyhemoglobin measurements taken by medical response and hospital personnel. b. General Laborer, was exposed to an 8-hour time-weighted average of 205.3 ppm, based upon percent carboxyhemoglobin measurements taken by medical response and hospital personnel. THIS ITEM WAS CORRECTED DURING INSPECTION.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): On or about January 30, 2012, workers at the Millard Refrigerated Services, Inc., facility located in Geneva, Illinois, were exposed to carbon monoxide levels which exceeded the permissible exposure limit prescribed in 29 CFR 1010.1000(a). Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the prescribed limit. THIS ITEM WAS CORRECTED DURING INSPECTION.
