29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):  a. Where equipment such as, but not limited to, blow molding machines, grinders, trim saws, and other saws, were repaired, maintained, and unjammed, specific procedures did not include details on energy isolating means, how to de-energize, location of the energy isolation device to be used for lockout, and specifically how to test the equipment.  Lockout procedures had not been developed for all equipment, such as for robots.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

Where equipment such as, but not limited to, blow molding machines, grinders, trim saws, and other saws, were repaired, maintained, and unjammed, an annual inspection audit was not certified.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
