29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use: (a) Custom On-Site, Inc. @ 4735 Dillion Ave., Loveland, Colorado 80538: On and before January 26th, 2012, the creating and exposing employer did not ensure that a platform connected to a rough terrain forklift, and used to raise an employee, had been approved by the forklift manufacturer for such use. The employer used a rough terrain forklift manufactured by Gradall, to raise an employee in a metal scaffold platform measuring 6' x 16'. The width of the forklift wheelbase measured approximately 90 inches, and the 16' wide platform exceeded the forklift wheelbase by approximately 51 inches. This use resulted in an unstable platform, and the condition exposed the employee to a fall hazard approximately 11 feet from the platform to the ground. OR IN THE ALTERNATIVE 29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design: (a)Custom On-Site, Inc. @ 4735 Dillon Ave., Loveland, CO 80538: On and before January 26th, 2012, the creating and exposing employer did not ensure that a platform connected to a rough terrain forklift, and used to raise an employee, had been designed by a qualified person and constructed and loaded in accordance with that design.
