29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Pappi & Son Roofing, conducting work at 131 Brandemere Ct. Elyria, OH: On or about February 2, 201, the employer failed to ensure that employees were wearing appropriate eye protection such as, but not limited to, safety glasses while securing shingles to the roof with Bostitch pneumatic nail gun. Employees were exposed to the hazards of eye injury from flying objects. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Elyria, OH: On or about February 2, 201, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing 12/12 pitch roof of the two story residential building. Employees did not use fall protection and were exposed to fall hazards of over 20 feet from the roof to the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Pappi & Son Roofing, conducting work at 131 Brandemere Ct. Elyria, OH: On or about February 2, 201, the employer did not ensure that employees, using an extension ladder to access the roof of the two story residential building in order to conduct roofing activities, were trained to recognize the hazards associated with the ladders use and to understand the procedures to control or minimize those hazards. Employees did not extend the ladder at least 3 feet above the roof edge and were exposed to approximately a 20 foot fall hazard from the ladder to the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
