29 CFR 1910.1048(d)(2)(i): Unless the employer chooses to measure the exposure of each employee potentially exposed to formaldehyde, the employer shall develop a representative sampling strategy and measure sufficient exposures within each job classification for each work shift to correctly characterize and not underestimate the exposure of any employee within each exposure group. a. On or about February 7, 2012, in the morgue, the employer conducted formaldehyde sampling when no employees were working in the area. Therefore the results were not representative of normal employee exposure as required by 29 CFR 1910.1048.

29 CFR 1910.1048(i)(3): If there is any possibility that an employee's eyes may be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer shall provide acceptable eyewash facilities within the immediate work area for emergency use. a. On or about February 2, 2012, in the surgery area, employees required to work with fromaldehyde were not provided with suitable facilities for quick drenching or flushing of the eyes and body.
