29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about February 8, 2012, at the above addressed jobsite, employees had not received training in the hazards of working in an unprotected excavation and the required precautions needed to protect themselves from those hazards.

On or about February 8, 2012, at the above addressed jobsite, an employee working in the 5 foot 6 inch deep excavation was exposed to potential hazards of wall collapse due to the lack of sloping, shoring, benching and/or shielding.
